From Grassroots to Groundbreaking

Achievable Health began as a grassroots movement—led by parents determined to improve life for families like their own, navigating the challenges of intellectual and developmental disabilities (I/DD).

What started as an urgent response to financial and social hardship has grown into a groundbreaking healthcare organization, delivering specialized, patient-centered care.

Today, Achievable Health is a Federally Qualified Health Center (FQHC) transforming how healthcare is delivered to people with I/DD and other medically vulnerable individuals. Our integrated model combines primary care, behavioral health, and wraparound support—meeting people where they are and addressing the full spectrum of needs that shape health and quality of life.

We’re not just closing gaps—we’re reimagining what healthcare can be, so that individuals and families have the opportunity to live healthier, more fulfilling lives.
